Mission Statement

To provide effective and ethical services that will help advance the social and psychological well-being of individuals with developmental disabilities by conducting myself in an ethical manner, and to help individuals make ethical decisions by documenting, engaging, assessing, intervening, and using knowledge and skills to help individuals retain a better understanding, achieve goals, and complete forms, while working with communities, families, groups, and organizations, so that each individual is provided an opportunity to enhance and improve their quality of life.

What We Do

1. A staff member is considered a support coordinator, who works with an individual to help them gain access to the community and assist in coordinating services and support in which the individual wants or needs that builds upon the individual’s capacity to engage in activities that promote community life and honor the individual’s preferences, choices, and abilities.

2. It is the responsibility of the support coordinator to assure that the individual’s support plan is developed utilizing a person-centered planning process. The methods of gathering information or evidence may vary and include the review of administrative documents, APD policy and guidelines, case record reviews and interviews/focus groups, individuals, and/or their families.

3. Support Coordinators are required to provide detailed documentation/notes of each individual on their caseload each week, to track their progress, services received, and their development.

4. Support coordinators do not provide money management or budgeting assistance to an individual. If an individual requires money management or budgeting assistance the support coordinator will refer the individual to their support living coach, guardian, and/or family member.
